When you use this form, it is assumed your event will not be communicated to the church family, there will be no room setup/arrangement and only a tv cart will be available for technology

Complex Ministry Form

Use this form when more extensive resources and coordination are needed.

Use this form if your event/activity needs any of the following: communication to the church family, a particular set up or arrangement in a room, registrations/signups, food and/or more than a tv cart for technology.

Vehicle Request Form

Use this form to reserve one of the church vans.

Davisville vehicles are only to be used for church ministry purposes. Our vans are not available for personal use or outside organizations.
